Case Planner (Bilingual Preferred)

Hope For Youth is actively seeking and scheduling interviews for qualified Spanish/English-speaking candidates to fill several?Case Planner roles.

In accordance with agency policy and procedures,the?overall focus of Case Planners will be to improve individual functioning and family dynamics to prevent out of home placement for the youth.Case Planners will work?flexible schedules?and have?most of their work hours in the field.?It is anticipated and expected that these case managers will be meeting with families in the?evening and on weekends. Each Case Planner will have a?caseload of 15-25. In addition, Case Planner responsibilities include:

  • Maintaining regular contact with the youth and their families at least once per week and implementing the various parts of the program model.

  • Providing case management services, life coaching, parent skill training, and make all collateral contacts as necessary.

  • Regularly reviewing the service plan with the youth and family.

  • Meeting weekly with their supervisor to review the cases. Maintaining the case record which will include completing progress notes contemporaneously with the events.

  • Completing the initial CANS-NY and all subsequent CANS-NY, as well as attending school meetings and court appearances with the families as necessary and be responsible for discharge/transition planning.

Spanish-speaking is required for a minimum number of these openings.

  • BSW (or Bachelor's in related field) plus 2 years related casework expereince or MSW

  • Spanish and English fluency preferred?

  • Excellent written, verbal and computer skills

  • Flexible schedule to accommodate program

  • Participate in treatment team, case review, and other meetings

  • Work collaboratively with referral resources and any other agency working with the family and the court

  • Work as a part of an interdisciplinary team

  • Provide support for other projects and locations; travel as needed

  • Knowledge of child serving systems

  • Experience with adolescents necessary

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
